Client expressed positive thoughts about school saying he was going to be good. Client reviewed his morning assignment with QMHP and is seen completing it without complaining. Client was given a thumbs up for having a positive attitude and being productive. Client displayed cooperative behaviors while listening to teacher give prompts to the class. Client was observed in group setting interacting well with peers. Client takes initiative to help others during group activity. Client symptoms of ADHD have been decreasing due to avoiding distractions. Client transitions to speak with mental health professional and attend lunch. (PM)Client was given permission to sit with counselor. Teacher reported client completed all of his work and is having a good morning. Client expresses positive and pleasant feelings toward peers and authority, at this time. Client engages in group discussion. Client shares his daily experiences at school in order to reflect and debrief during session. Client inquired about group activity. Client has implemented interventions and strategies taught by QMHP in his daily activities. Client shows great attitude and high level of participation with his peers. Client complies to instruction and is seen working as a team. Client communicates well in group setting, expressing positive thoughts about activity. Client works well with others. Client understands his expectations and leads by example. (EOD) Client is observed displaying age appropriate manners, avoiding distracting behaviors. Client was given positive feedback regarding the situations in which he has been using his new prosocial behaviors in the group setting. Client continues to build new healthy relationships in school and rapport with QMHP.
